Five Islands is located on the west coast of Nova Scotia (on the Minas Basin, at the northeast end of the Bay of Fundy).
While we don't have a date for the founding of Five Islands, you might get a feel for it by knowing that the earliest mention that we've found (so far) was on a map dated 1897.
